On behalf of the __(Name of Organization – e.g. PTA)__, I am writing this letter to levy a formal complaint against principal, __(full name of principal)__. Over the past __(#)__ __(years/months/weeks)__, parents and teachers of the students enrolled in __(Name of School)__ have observed that __(Mr./Ms/Miss/Mrs. Name)__ seriously lacks the ability, vision and leadership that this school needs from its principal.
