I wish to make you aware of a situation that occurred on our account with __(name of company)__ and request your assistance in bringing this to a quick resolution.
On __(date)__, it was brought to my attention that __(briefly describe situation)__. When I personally spoke to __(name)__, in your __(type department/branch/etc)__, about the situation, __(he/she)__ __(briefly describe what he/she said or did)__. This __(behavior was completely uncalled for/was a poor attempt at resolving the situation/etc.)__ and is completely unacceptable. This is not the first clash we have had with __(Name)__ and, needless to say, I am not happy with the poor way in which __(he/she)__ handles our account.
Consequently, I request that __(he/she)__ be immediately removed from our account as __(Account Manager/Account Executive/Customer Service Representative/etc.)__. It is important for our organization to be comfortable with the individual who looks after our account and, unfortunately, our comfort level has now been breached. As there is no longer a rapport between __(Name)__ and __(Our Organization’s Name)__, I ask that you please find a suitable replacement to service our account.
